EOI forms will typically ask your age, birthdate, full name, address, height, weight and blood pressure. They will ask if you take any medications and if you have any current medical conditions. The form will also ask you if you’ve ever had any surgeries or procedures and to list those medical procedures and surgeries. These forms usually ask about your parents’ and siblings’ medical histories. Lastly, you will be asked about your lifestyle. For example, you’ll be asked if you smoke, drink or use drugs and how often you exercise or your activity levels.
